IMPLEMENTATION STRATEGY OF AYUSHMAN BHARAT
- At the national level to manage, an Ayushman Bharat National Health Protection Mission Agency (AB-NHPMA) would be put in place. States/ UTs would be advised to implement the scheme by a dedicated entity called State Health Agency (SHA). They can either use an existing Trust/ Society/ Not for Profit Company/ State Nodal Agency (SNA) or set up a new entity to implement the scheme.
- States/ UTs can decide to implement the scheme through an insurance company or directly through the Trust/ Society or use an integrated model.

- Participation of States
The government proposed a 40% participation of states in contributing towards premium and implementation of Ayushman Bharat. However, till date, there has been no communication to states. Some of the states might already be having similar schemes implemented. For example, Karnataka government has plans to roll out Universal Health Coverage (UHC) which intend to provide health coverage of approximate Rs. 1.5 Lacs to 1.4 crore households living in BPL category. This cashless facility can be availed in Public and Private Hospitals both.
- Poor infrastructure
Additional coverage provided is expected to increase demand for healthcare service. This, in turn, is going to increase patients in private as well as public hospitals. However, most of the hospitals especially in remote areas do not have infrastructure and resources to respond.

The above graph shows, there are 13.5 lakhs existing hospital beds (pre NHPM) and post NHPM there is demand for more 1.6 lakhs hospital beds in India.
- Low participation from the private sector
Private hospitals may try to exclude themselves from accepting the health coverage provided by the government in Ayushman Bharat scheme. Private hospitals providing medicare for such benefits are unhappy due to delays in payments or follow-up needed to be done with government machinery.
